Outcome Engenuity and the South Carolina Hospital Association (SCHA) announce an intervention to reduce the rate of mislabeled blood specimens by 90% in 90 days. It's called The Final Check. The Final Check is based on the findings from an independent Probabilistic Risk Assessment of the human factors and system contributors that lead to mislabeled specimens. The intervention was initially piloted at Palmetto Health Richland in South Carolina, and subsequently adopted by five other hospitals that were recruited by the SCHA-- all of which have now demonstrated and sustained a 90-93% drop in the number of mislabeled blood specimens over the last five months.
